Postherpetic Neuralgia: The Rash Is Gone, but the Pain Stayed|

A band of blisters broke out on your waist or back; the rash faded, yet that patch of skin still stings and burns in waves — even a light touch of clothing feels unbearable. Many people only learn then: the hardest part of shingles isn't the rash, it's the nerve pain it leaves behind.

What Western Medicine Says

Shingles is the reactivation of the varicella-zoster virus, "burning" a rash along a nerve. The critical window is within 72 hours of onset: the earlier antivirals start, the shorter the course and the lower the risk of lingering neuralgia — the single most important sentence in shingles care. Don't wait and watch a suspicious rash; seek care immediately.

Even so, some people (especially those over 60) develop postherpetic neuralgia (PHN): the virus damaged sensory nerves, and the "short-circuited" nerves keep misfiring pain signals. First-line treatment is gabapentinoids/pregabalin for neuropathic pain, plus topical lidocaine patches, with pain specialists managing severe cases. PHN has two hallmark features: hyperalgesia (a light touch hurts) and allodynia (stimuli that shouldn't hurt do hurt) — these mark nerve damage, not "psychosomatic" pain, and certainly not "being dramatic." An honest framing: PHN is recognized as one of the hard-to-treat pains — the goal is often "pain low enough to live with," not zero.

What TCM Sees

TCM calls shingles "she chuan chuang" (snake-string sores) or "chan yao huo dan" (waist-wrapping fire cinnabar), with a core mechanism of "damp-heat fire toxin with blood stasis blocking the collaterals." Staged treatment is key:

  • Acute stage (rash and blisters present): focused on clearing heat, draining dampness, detoxifying, and stopping pain — but Western antivirals lead at this stage; TCM only supports, never takes over.

  • Postherpetic neuralgia stage (rash gone, pain remains): the mechanism shifts to "lingering toxin with blood stasis blocking the collaterals," or chronic disease entering the collaterals with qi and blood deficiency. Two common patterns:

  • Blood stasis blocking the collaterals: fixed, stabbing pain, worse at night, darkened skin or pigmentation over the area. Approach: invigorate blood, resolve stasis, open the collaterals to stop pain.

  • Dual deficiency of qi and blood: prolonged illness with depletion — dull lingering pain that waxes and wanes, with fatigue and poor sleep. Approach: tonify qi, nourish blood, open the collaterals to stop pain.

TCM pays special attention to the "collaterals" in PHN — chronic disease enters the collaterals, so treatment takes patience; improvement is usually slower than with acute conditions and should be judged over a full course. TCM also stresses that "chronic disease always involves stasis": beyond three months, invigorating blood and opening the collaterals is nearly always indicated; yet "chronic disease injures the upright qi," so tonifying deficiency and resolving stasis must go together — never attack blindly.

Typical Presentations

  • Two to three months after the rash faded, the area still stabs with pain, worse at night, skin darkened — blood stasis blocking the collaterals

  • Electric-shock, burning pain; clothing friction is intolerable; bearable by day, worse every night — blood stasis with lingering toxin

  • Over six months in: dull unrelenting pain, weight loss, fatigue, pain wrecking sleep — dual deficiency of qi and blood (chronic disease in the collaterals)

How Acupuncture Helps

During the acute shingles stage (rash and blisters), Western antiviral treatment leads and acupuncture is not the main approach; acupuncture is aimed at the postherpetic neuralgia stage. Pattern-based point selection: blood-invigorating, collateral-opening points for stasis patterns (often around the affected area and corresponding nerve segments), qi-tonifying, blood-nourishing points for deficiency — aiming to ease neuropathic pain and improve sleep and quality of life. Some clinical studies suggest acupuncture may improve PHN pain scores, but most have methodological limitations and the evidence quality is limited. PHN is best managed with an East–West combination: keep standard medications going, add acupuncture as an adjunct — the two work better together.

5 Things You Can Do Daily

Wear loose, soft clothing over the area to reduce friction; keep regular hours — pain and sleep feed each other, so protect sleep first; gently move the affected limb to avoid stiffness from guarding it; when pain flares, a warm towel compress for 15 minutes can help, but don't scratch or pick at the skin — intact skin is the first line of defense against infection, and don't share towels with family during the blister stage; keep a "pain diary" (timing, triggers, intensity) to show your doctor at follow-ups; families, please understand — nerve pain is invisible, and what sufferers need most is not being dismissed as "dramatic."

Time to See a Doctor If…

A suspicious one-sided cluster of blisters appears — seek care immediately (remember the 72-hour antiviral window); pain persists more than a month after the rash fades; pain disrupts sleep and daily life; or medication side effects need adjustment. PHN can run a long course — early intervention and sticking with a full course of treatment brings gradual improvement for most. If you're over 50, ask your doctor about the shingles vaccine; the recombinant vaccine significantly lowers the risk of shingles and PHN.
