Sciatica: A Live Wire from Your Back to Your Foot|
The pain doesn't stay in your back — it shoots from the buttock down the back of the thigh to the calf, even the foot: electric, burning. Sitting too long leaves you unable to stand straight; even a cough or sneeze tugs at it. That's sciatica — the pain travels a line, and the trouble usually starts at its origin.
What Western Medicine Says
The sciatic nerve is the body's thickest nerve, running from the lumbar spine through the buttock and down the back of the thigh to the foot. The most common cause is a herniated lumbar disc: the nucleus pulposus bulges backward and compresses a nerve root, sending pain radiating along the nerve's path. Other causes include spinal stenosis and piriformis syndrome — the piriformis muscle in the buttock entrapping the sciatic nerve, which mimics a disc problem even when the lumbar spine itself is fine. Standard care: NSAIDs and brief rest in the acute phase, plus physical therapy such as McKenzie exercises; most people improve on their own within 6 to 12 weeks. Epidural injections or surgical evaluation are considered only for stubborn pain or progressive weakness.
What TCM Sees: Identify Your Pattern First
TCM classifies sciatica under "bi syndrome" and "lumbar-leg pain," emphasizing differentiation by channel and pattern. Three common patterns in practice:
Cold-damp obstruction: heavy pain that worsens with cold and eases with warmth, with cold, heavy legs; treatment focuses on scattering cold, removing dampness, and warming the channels;
Qi stagnation and blood stasis: stabbing, fixed pain worse at night, aggravated by prolonged sitting or standing, often with a history of injury or chronic strain; treatment focuses on invigorating blood and moving qi to open the channels;
Liver and kidney deficiency: long course with recurrent episodes, sore weak lower back and knees, leg weakness worse with exertion — common in middle-aged, older, or chronically ill patients; treatment focuses on tonifying liver and kidney to strengthen sinews and bones.
Cold-damp obstruction and qi-blood stasis are the most common at onset; chronic cases usually add liver-kidney deficiency — prolonged blockage depletes the foundation.
Typical Presentations
Deep buttock ache shooting down the back of the thigh to the calf; can't sit longer than half an hour — usually cold-damp obstruction or piriformis-type entrapment
Leg numbness after a back sprain, with electric shocks down the leg when coughing or sneezing — usually qi stagnation and blood stasis irritating the nerve root
Recurrent leg pain for years, worse on rainy days, with a weak aching back — usually liver-kidney deficiency with cold-damp
One-sided buttock numbness after long sitting, easing after a few steps — usually early qi stagnation and blood stasis
How Acupuncture Helps
Acupuncture for sciatica follows channel-based pattern differentiation: points along the bladder and gallbladder channels that trace the sciatic nerve's path, to move local qi and blood; moxibustion added for pronounced cold-damp, blood-invigorating emphasis for stasis; chronic cases also tonify liver and kidney to support the foundation. Systematic reviews suggest acupuncture may improve pain scores and function in sciatica, but study quality is uneven and sample sizes vary, so the evidence quality is limited and cannot replace necessary imaging. Progressive leg weakness, foot drop, saddle numbness, or bowel/bladder dysfunction — warning signs of cauda equina syndrome — require immediate medical care; don't let them delay surgical timing.
5 Things You Can Do Every Day
Avoid long sitting — get up every 40 minutes, and use a lumbar roll to preserve the lower back's natural curve when seated;
Sleep with a pillow between your knees (side-lying) or under your knees (on your back) to reduce nerve traction;
During acute flare-ups, avoid bending to lift and hard benches; resume exercise gradually only after pain eases;
Do 5 minutes of piriformis stretching daily (supine knee hug, figure-4 stretch) to release the deep buttock muscles;
Keep the lower back and buttocks warm; if you run cold-damp, avoid long sitting in cold, damp places and go easy on cold foods in summer.
Time to See a Doctor If…
Leg pain or numbness lasts more than 6 weeks; you develop foot drop or a dragging gait; saddle numbness or bowel/bladder changes — that's an emergency; or pain severely disrupts sleep and daily life. Rule out the cases that need surgery first, then pursue conservative care with confidence.

寒湿痹阻:疼痛沉重、遇冷加重、得热则舒,下肢发凉沉重,治法以散寒除湿、温经通络为主;
气滞血瘀:疼痛如针刺、部位固定、夜间加重,久坐久站后明显,多有外伤或慢性劳损史,舌质紫暗或有瘀点,治法以活血化瘀、行气通络为主;
避免久坐,每 40 分钟起身活动,坐时腰后垫个靠枕保持腰椎前凸;
睡觉时在两膝之间夹枕头(侧卧)或膝下垫枕(仰卧),减少神经牵拉;
急性发作期避免弯腰搬重物、避免久坐硬板凳,疼痛缓解后再循序渐进锻炼;
每天做 5 分钟梨状肌拉伸(仰卧抱膝、"4"字拉伸),松解臀部深层肌肉——很多"假性坐骨神经痛"就卡在这里;
