Menopause: Hot Flashes Aren't Just "Getting Older"|
Waking up drenched in night sweats; a sudden wave of heat and flushing mid-conversation that passes in minutes. If this sounds familiar, this one's for you — especially if you're in perimenopause.
What Western Medicine Says
Perimenopause is the gradual decline of ovarian function with fluctuating, falling estrogen, typically beginning between ages 45 and 55 and lasting several years. Hot flashes, night sweats, insomnia, mood swings, vaginal dryness, and bone loss all trace back to these hormonal shifts. Menopausal hormone therapy (MHT) is currently the most effective treatment for moderate-to-severe hot flashes, but whether it's right for you requires a doctor to weigh risks and benefits around breast health, blood clots, and cardiovascular health. Non-hormonal medications and lifestyle changes can help too.
What TCM Sees
TCM describes this stage as the gradual decline of kidney qi — the kidney is the through-line. In my practice, four patterns are most common:
Kidney yin deficiency (yin deficiency with ascendant fire): hot flashes, night sweats, heat in the palms and soles, dry mouth, restless sleep with vivid dreams — approach: nourish kidney yin and clear deficiency fire.
Kidney yang deficiency: cold intolerance, sore achy lower back and knees, frequent nighttime urination, low energy — approach: warm and support kidney yang.
Liver qi stagnation / liver fire: mood swings, irritability, chest tightness, breast tenderness, difficulty falling asleep — approach: soothe the liver and regulate qi, calm liver fire.
Non-interaction of heart and kidney: restlessness, palpitations with hot flashes, forgetfulness — approach: harmonize heart and kidney.
Most people have a mix — kidney deficiency as the root, liver stagnation as the branch — which is why treatment has to be staged around the dominant pattern. One important note: any vaginal bleeding after menopause must first be checked by a gynecologist to rule out structural problems; it can't simply be chalked up to "menopause."
Typical Presentations
Hot flashes 4–5 times a day, night sweats soaking through sleepwear, then lying awake for hours — kidney yin deficiency
Hair-trigger temper, chest tightness and breast tenderness, periods getting erratic — liver stagnation / liver fire
Achy lower back, cold intolerance, 2–3 nighttime bathroom trips, dragging through the day — kidney yang deficiency
How Acupuncture Helps
Acupuncture uses pattern-based point selection to harmonize yin and yang and calm the mind. Research suggests it may ease hot flash frequency, sleep quality, and mood symptoms, but most studies have methodological limitations and the evidence quality is limited. Acupuncture can't replace hormone therapy's protective effect on bone density — bone and hormone evaluations still belong with your doctor.
5 Things You Can Do Daily
Get 150 minutes of moderate exercise a week (brisk walking and swimming count); cut back on spicy food, alcohol, and coffee; dress in layers and keep the bedroom cool and ventilated; make sure you're getting enough calcium and vitamin D; spend 10 minutes a day on deep breathing or meditation to cool down your nervous system.
Time to See a Doctor If…
Hot flashes and night sweats are wrecking your sleep or work; low mood lasts more than two weeks; you have any vaginal bleeding after menopause; or you want to know whether hormone therapy is right for you. Rule out structural problems first, then talk about regulation — in that order.
