Migraine: Not Just a Headache — Life on Pause|
One temple throbs as if clamped in a vise, pulsing with each heartbeat; light and noise become unbearable, and the smell of cooking oil turns your stomach. In a bad attack, all you can do is lie in a dark room, everything on hold. One migraine can force life to "pause" for hours, even days.
What Western Medicine Says
Migraine isn't an ordinary headache — it's a neurovascular disorder: the trigeminovascular system activates, releasing inflammatory mediators that change cerebral blood vessel tone and trigger neurogenic inflammation. That's why the pain is typically throbbing, often with nausea, light and sound sensitivity. About a third of people get an aura beforehand — flashing lights, blind spots, tingling hands. Triggers are personal: disrupted sleep, sudden stress shifts, menstrual hormone swings, red wine, aged cheese, chocolate, bright lights, strong smells. First-line acute treatment is triptans (constricting dilated vessels, blocking pain signals); preventive medication is for frequent attacks (more than 4 days a month). What must be ruled out first is secondary headache: a sudden "worst headache of my life," fever with neck stiffness, or new headache after 50 — all need imaging.
What TCM Sees: Identify Your Pattern First
TCM classifies migraine under "headache" and "head wind," with the guiding principle that "pain comes from blockage or from malnourishment." Three common patterns in practice:
Liver yang rising: severe distending pain with dizziness, flushed face, irritability, triggered by emotional upset or overwork; treatment focuses on calming the liver, subduing yang, clearing heat, and extinguishing wind;
Qi and blood deficiency: a dull lingering ache that worsens with exertion and eases with rest, with palpitations, fatigue, and a pale face; treatment focuses on tonifying qi and nourishing blood to feed the head;
Blood stasis obstructing the channels: fixed stabbing pain, long course with recurrent attacks, dark-purple tongue; treatment focuses on invigorating blood and opening the channels to stop pain.
Many migraines are "deficiency at the root, excess on the surface": depleted qi and blood underneath, with liver yang surging at the slightest trigger. Menstrual migraines often involve liver qi stagnation and disharmony of the thoroughfare and conception vessels.
Typical Presentations
Throbbing severe pain at one temple, light and noise sensitivity, nausea — sleeping it off helps: usually liver yang rising
A dull all-day headache, worse after overtime and late nights, with a pale face and no energy: usually qi and blood deficiency
Fixed needle-like pain at one spot, years of history, painkillers working less and less: usually blood stasis obstructing the channels
Headache arriving punctually around menstruation, with breast tenderness and irritability: usually liver qi stagnation with blood stasis
How Acupuncture Helps
Acupuncture for migraine follows pattern-based point selection: liver-yang-rising patterns focus on calming the liver, extinguishing wind, and coursing the shaoyang channel; qi-blood-deficiency patterns tonify qi and blood, regulating the spleen and stomach as the source of nourishment; blood stasis patterns invigorate blood and open the channels. During an attack, acupuncture aims to relieve pain quickly; between attacks, it regulates the constitution to reduce attack frequency. Systematic reviews suggest acupuncture may reduce migraine frequency and pain intensity, but included studies vary widely in point selection and treatment courses, so the evidence quality is limited and it cannot replace standard acute medication. Sudden severe headache, fever with neck stiffness, altered consciousness, or new headache after 50 suggest possible secondary causes — get imaging first.
5 Things You Can Do Every Day
Keep a headache diary: timing, duration, and possible triggers (sleep, food, menstrual cycle) to find your own;
Keep sleep regular with fixed bed and wake times — disrupted sleep is one of the most common triggers;
At the first sign of aura, retreat to a quiet dark place; try cold on the forehead or heat on the neck — see which works for you;
Avoid your known personal triggers: red wine, aged cheese, chocolate, MSG, bright lights, strong scents;
Spend 10 minutes daily on neck-shoulder release and deep breathing to stop chronic tension from fanning the flames.
Time to See a Doctor If…
Headaches strike more than 4 days a month and painkillers keep escalating; the headache pattern suddenly changes or worsens; or you get a "worst headache of my life," fever with neck stiffness, or vision/speech problems. Migraine can be managed — but rule out dangerous secondary headaches first.
Book a consultation — let's find the right plan for you.

肝阳上亢:头痛剧烈、胀痛为主,伴头晕、面红、易怒,情绪激动或劳累后诱发,治法以平肝潜阳、清热熄风为主;
气血亏虚:隐隐作痛、绵绵不休,劳累后加重、休息减轻,伴心悸、乏力、面色苍白,治法以益气养血、荣养头目为主;
记录"头痛日记":发作时间、持续时长、可能诱因(睡眠、饮食、经期),找到自己的触发器;
保持规律睡眠,固定起床和入睡时间——睡眠紊乱是最常见的诱因之一;
发作前兆一出现就到安静避光处休息,冷敷前额或热敷颈部,看哪种更管用;
少碰已知的个人诱因:红酒、陈年奶酪、巧克力、味精、强光和浓烈气味;
