Allergic Rhinitis: Not a Cold — Your Nose Is Having an Allergy|

Every pollen season brings sneezing fits and a faucet-like runny nose; some people are congested year-round, can't smell properly, and sleep poorly. Cold medicine doesn't help — because this isn't a cold at all. It's your nose having an allergic reaction.

What Western Medicine Says

Allergic rhinitis is an IgE-mediated type I hypersensitivity reaction: when allergens (pollen, dust mites, pet dander) enter the nose, the immune system overreacts, releasing histamine and other inflammatory mediators — cue the sneezing, watery discharge, itching, and congestion. It often teams up with asthma and allergic conjunctivitis, a concept medicine calls "one airway, one disease."

First-line treatment is clear: intranasal corticosteroids are the most effective drugs for controlling nasal inflammation; oral or intranasal antihistamines ease sneezing and runny nose; saline nasal irrigation helps too. One common myth needs clearing up: nasal steroid sprays act locally with minimal systemic absorption — used as directed long-term, they are safe. "Steroid spray" is not the same as "steroid pills that cause weight gain." Allergic rhinitis also impairs sleep and concentration, and chronic mouth breathing in children may even affect facial development — so "just tough it out" is not a good strategy. For confirmed allergens like dust mites, desensitization (immunotherapy) is also an option.

What TCM Sees

TCM groups allergic rhinitis under "bi qiu" (nasal discharge disorder), rooted in deficient upright qi letting wind-cold (or wind-heat) invade. Three common patterns in practice:

  • Lung qi deficiency with cold: the most common — cold intolerance, catching colds easily, congestion with clear discharge and sneezing, worse with cold exposure. Approach: tonify qi, secure the exterior, warm the lungs and open the nasal orifices. Many patients are the "air-conditioning sets it off" type.

  • Spleen qi deficiency: persistent congestion with poor appetite, loose stools, and easy fatigue; common in children and weaker constitutions. Approach: strengthen the spleen, tonify qi, raise yang to open the orifices — TCM says "the spleen is the source of phlegm production," and phlegm-dampness rising to the nose when the spleen is weak.

  • Kidney yang deficiency: long history with years of recurrence — bursts of morning sneezing with clear discharge, plus achy lower back and knees and cold intolerance. Approach: warm and tonify kidney yang, secure the exterior and open the orifices.

TCM also offers timing wisdom: rhinitis feels worst in winter, but the best time to treat it is summer (the sanfu dog days) — "treating winter disease in summer," building the foundation when yang qi peaks.

The three patterns are really one line: the lungs govern qi, the spleen generates qi, the kidneys grasp qi — lung qi deficiency is the branch, spleen–kidney deficiency the root. The longer the history, the more the cause lies with spleen and kidney — TCM's explanation for why old rhinitis is hard to uproot.

Typical Presentations

  • Sneezing fits and endless watery discharge each pollen season, itchy eyes too, flaring with any cold draft — lung qi deficiency with cold (wind-cold invading the lungs)

  • Year-round congestion, dulled sense of smell, with poor appetite, loose stools, low energy — spleen qi deficiency (spleen deficiency with phlegm obstruction)

  • A dozen sneezes in a row every morning on waking with clear discharge, years of history, plus lower back ache and cold intolerance — kidney yang deficiency

How Acupuncture Helps

Acupuncture uses pattern-based point selection: qi-tonifying, exterior-securing points for lung deficiency; spleen-strengthening points for spleen deficiency; warming points for kidney yang deficiency — with summer reinforcement following the "winter disease, summer treatment" principle — aiming to relieve congestion, sneezing, and runny nose and rebalance immune overreaction. Some clinical studies suggest acupuncture may improve allergic rhinitis symptom scores, but most have methodological limitations and the evidence quality is limited. Local points around the nose are often used for immediate congestion relief, while body acupuncture works on the overall constitution — the two are commonly combined. Severe acute flare-ups are still best controlled with standard medications like intranasal steroids; those with coexisting asthma or worsening symptoms should complete allergen testing and specialist evaluation first, with acupuncture as an adjunct.

5 Things You Can Do Daily

During pollen season, keep windows closed, run an air purifier, wear a mask outdoors, and wash your face and rinse your nose when home; sunglasses outdoors also cut pollen irritation to the eyes; hot-wash bedding weekly, de-mite, and air pillows in the sun; irrigate with saline once daily; cut back on cold/raw foods and cold drinks, and don't set the AC too low; keep pets out of the bedroom and groom them regularly; ventilate newly renovated homes well — irritants like formaldehyde also aggravate rhinitis; keep regular hours and exercise moderately — building constitution is the foundation.

Time to See a Doctor If…

Congestion is severe enough for chronic mouth breathing, smell is noticeably dulled, asthma (chest tightness, wheezing) or recurrent sinusitis tags along, or medications still don't control symptoms. Get allergen testing first to identify the "enemy" — skin-prick or blood specific-IgE testing pinpoints your triggers — then we talk treatment: know yourself and the allergen, and rhinitis gets better faster.
