Irregular Periods: When Your Cycle Goes Off Script — It's Not Just "Stress"|
Ten days early this month, half a month late the next; flow heavy then scanty, sometimes dark. When the cycle goes haywire, most people's first thought is "I've been stressed" — but the causes may run deeper than that.
What Western Medicine Says
Menstruation is orchestrated by the hypothalamic-pituitary-ovarian axis — a glitch anywhere along it can throw things off. Common causes include ovulatory disorders (polycystic ovary syndrome being the most common — its diagnosis needs two of three: irregular ovulation, polycystic-appearing ovaries on ultrasound, and signs of excess androgens, not one ultrasound report alone), thyroid dysfunction, and elevated prolactin; structural issues like endometriosis, uterine fibroids, and endometrial polyps or lesions must not be missed either. Stress, crash dieting, intense exercise, and rapid weight change can indeed disrupt cycles — but blaming everything on "stress" delays real diagnoses. Irregularity is relatively common in the 1–2 years after menarche and around perimenopause; persistent chaos during the reproductive years deserves a systematic workup. The basic panel: pelvic ultrasound for the endometrium and ovaries, sex hormones for ovulation status, and thyroid function to rule out thyroid issues — those three usually pin down the direction.
Standard care starts with testing: pelvic ultrasound, sex hormones, and thyroid function are the basics; lifestyle adjustment (weight, exercise, sleep) is first-line; combined oral contraceptives or progestins regulate the cycle when needed. One red flag to remember: any postmenopausal bleeding or bleeding between periods needs prompt evaluation — never watch and wait.
What TCM Sees: Reading Timing, Volume, Color, and Texture
TCM reads irregular periods through four lenses — timing, volume, color, and texture. Four patterns are common:
Liver qi stagnation: cycles swinging early or late with stress and mood shifts, unsmooth flow, pre-period breast and rib-side distension, irritability, and frequent sighing; treated by soothing the liver and regulating qi.
Spleen deficiency: heavy flow, prolonged periods, pale thin blood, plus fatigue, poor appetite, and profuse white discharge; treated by strengthening the spleen, boosting qi, and securing the thoroughfare vessel.
Kidney deficiency: delayed cycles, scanty pale flow, sometimes only every two to three months, plus lower-back soreness, cold sensitivity, and often dizziness, tinnitus, and nighttime urination; treated by tonifying the kidney and nourishing blood to regulate menstruation.
Blood stasis: dark-purple flow with clots, menstrual cramping that eases after clots pass, unsmooth flow varying in volume; treated by moving blood, dispelling stasis, and regulating qi.
Treatment follows the pattern — direction only here. Differentiating menstrual disorders properly requires combining the description with test results, never a conclusion from words alone. Differentiation comes down to four characters: timing (early/late), volume (heavy/scanty), color (pale/dark), and texture (thin/thick with clots).
Typical Presentations
Early when work gets busy, late when things calm down, breast tenderness and a short fuse before each period: commonly liver qi stagnation
Each period dragging seven or eight days, heavy flow, exhausted and pale: spleen deficiency
A period only every two or three months, scanty pale flow, sore back, cold sensitivity, icy hands and feet: kidney deficiency with stasis
How Acupuncture Helps
Phase-based point selection across the menstrual cycle is a common clinical approach: nourishing blood after the period, warming yang to support ovulation mid-cycle, soothing liver qi before the period, and moving blood during menstruation. Research suggests acupuncture may improve cycle irregularity related to polycystic ovary syndrome, but studies have methodological limitations and the evidence quality is limited. Amenorrhea beyond 3 months, intermenstrual bleeding, or postmenopausal bleeding needs gynecologic and ultrasound workup first to rule out structural disease before regulating.
5 Things You Can Do Every Day
Track your cycle: log start/end dates, flow, and symptoms in an app — three months reveals the pattern, and you can show it directly to your doctor; note weight changes and stressful events alongside it so you have the full picture.
Stabilize sleep: all-nighters directly disturb hormonal rhythms; keep a fixed bedtime, especially before your period.
Avoid extremes: crash diets and excessive exercise can stop periods altogether; lose weight gradually — too-low body fat disrupts cycles too; sharp short-term weight swings deserve the same caution.
Decompress: chronic high stress suppresses ovulation; find your own way to unwind — exercise, meditation, or time with friends all count.
Learn the alarm signals: cycles shorter than 21 days or longer than 45 days, bleeding between periods, or sudden marked changes in flow — don't delay, get checked, especially new-onset irregularity after age 35.
Time to See a Doctor If…
No period for over 3 months, bleeding between periods, any postmenopausal bleeding (must be checked), a year of trying to conceive without success, or cycle chaos with marked acne, excess hair growth, or hair loss (possible PCOS). The menstrual cycle is the body's barometer — when it's off, it deserves one thorough workup. Diagnose first, then regulate — that's how you avoid detours.
