Frozen Shoulder: When Your Arm Won't Go Over Your Head — It's Not Just a Sore Shoulder|

You can't reach behind your back to dress, can't lift your arm to comb your hair, and rolling onto that shoulder at night wakes you with pain. Around age fifty, don't dismiss this as "just tired" — your shoulder capsule may be slowly "freezing."

What Western Medicine Says

Frozen shoulder — adhesive capsulitis — happens when the shoulder capsule becomes inflamed, then thickens, adheres, and contracts, freezing the joint's range of motion bit by bit. The classic course runs in three stages: the painful stage (weeks to months, pain dominates), the stiff stage (months, immobility dominates), and the thawing stage (gradual recovery). The whole process is self-limiting but can last 1–2 years; people with diabetes or thyroid disease face higher risk and slower recovery. It favors ages 40–60, slightly more women; many cases follow a minor trauma or a period of shoulder immobilization (such as a post-surgery sling). Night pain is the hallmark: rolling onto it wakes you, and it's what brings many people in. The course may be self-limiting, but the longer it stays frozen, the more muscle wasting and secondary problems accumulate — waiting it out passively isn't advised.

Standard care: NSAIDs for pain control; physical therapy to preserve and restore motion is key; intra-articular injections can relieve symptoms short-term; only rare severe cases go on to manipulation under anesthesia or arthroscopic surgery. One caution: rotator cuff tears, cervical spine disease, and calcific tendinitis can all present as shoulder pain — don't diagnose yourself or push through aggressive exercise before it's sorted out. For differentiation: rotator cuff tears usually follow trauma and bring weakness lifting the arm, while cervical spine disease tends to add neck discomfort and radiating arm pain — history plus exam usually separates them, with ultrasound or MRI when uncertain.

What TCM Sees: Three Patterns

TCM classifies frozen shoulder as "shoulder bi syndrome," with the core mechanism being blocked channels and sluggish qi and blood. Three patterns are common:

  • Cold congealing with blood stasis: cold, fixed shoulder pain, worse with cold and eased by warmth; pain worse at night, often waking the patient; a pale tongue with white coating and cooler skin over the shoulder are frequently seen.

  • Qi stagnation with blood stasis: clear history of trauma or strain, fixed pain point tender to touch, marked limitation of movement, sometimes with local tender points; night pain with a fixed location is its signature.

  • Qi-blood deficiency: older or debilitated patients after prolonged illness, with sore, heavy, weak shoulders that lift with effort, plus fatigue and a dull complexion; the course often exceeds half a year, sometimes with dizziness and palpitations.

Treatment follows the pattern: warm the channels, dispel cold, move blood, and open the collaterals as the base; deficient cases add qi-blood and liver-kidney tonification — supporting the upright while opening flow. Differentiation looks at three things: any trauma history, cold/heat preference, and duration versus constitution.

Typical Presentations

  • Waking at night when rolling onto the shoulder, cold-sensitive shoulder, worse after air conditioning: commonly cold congealing with blood stasis

  • After wrenching it lifting something heavy, one spot on the shoulder exquisitely tender, arm lifting less than halfway: qi stagnation with blood stasis

  • In your sixties, shoulder stiff for over half a year, struggling to dress or button up, generally listless and weak: qi-blood deficiency

How Acupuncture Helps

Acupuncture uses local shoulder points such as Jianyu, Jianzhen, and Jianliao plus distal channel-based points, sometimes with electroacupuncture or warm needling to strengthen the channel-opening, pain-relieving, adhesion-loosening effect. Stage-based care is key: pain relief dominates the painful stage, acupuncture plus functional exercise run together in the stiff stage, and consolidation in the thawing stage. Research suggests acupuncture may improve shoulder pain and range of motion, but studies have methodological limitations and the evidence quality is limited. Sudden severe pain with trauma, visible deformity, or marked weakness lifting the arm (watch for large rotator cuff tears or dislocation) needs imaging first.

5 Things You Can Do Every Day

  1. Wall climbs: face a wall and "climb" your fingers upward, holding 10 seconds at your limit, several times daily, progressing gradually; keep it below the threshold of sharp pain — a sore, distended feeling is the right intensity signal.

  2. Pendulum swings: bend forward, let the affected arm hang, and draw small circles from the shoulder — one minute each direction.

  3. Heat: apply heat to the shoulder 15–20 minutes daily, ideally before exercising, for better loosening; a 5–10 minute hot shower stream over the shoulder works too.

  4. Don't overprotect: avoiding all movement for fear of pain only worsens adhesions — keep moving within a range that doesn't trigger severe pain.

  5. Adjust sleep position: avoid lying on the affected side; tuck a pillow under the armpit to keep the shoulder relaxed; add a small pillow under the affected arm so the shoulder doesn't hang under traction.

Time to See a Doctor If…

Shoulder pain shows no relief after 2–3 weeks, wakes you at night, the arm is markedly weak or feels "out of socket," or a diabetic patient's shoulder pain and stiffness keep worsening. Frozen shoulder may be self-limiting, but a 1–2 year "frozen" stretch is a long grind — early pattern differentiation and early care can clearly shorten the course and spare a lot of suffering.
