Chronic Fatigue: Slept Enough, Still Exhausted|

You sleep seven or eight hours a night, yet wake up feeling uncharged; two flights of stairs leave you winded, your eyelids droop at 3 p.m., and coffee works less and less — this kind of tiredness that no rest seems to fix deserves to be taken seriously.

What Western Medicine Says

Fatigue is the body's generic alarm, with many possible causes. Western medicine's first step with chronic fatigue is always "rule out first": check thyroid function (hypothyroidism drains energy), run a blood count (anemia is one of the most common correctable causes), assess for sleep apnea (repeated nighttime oxygen drops leave you tired no matter how long you sleep), plus blood sugar, liver and kidney function, chronic infections, and medication side effects. Notably, several common medications (some blood-pressure drugs, antihistamines) and chronic sleep debt can also cause fatigue — bring a complete medication list to your visit. Only after organic disease is excluded does the remaining "nothing shows up but I'm still exhausted" point toward diagnoses like chronic fatigue syndrome, managed mainly with lifestyle measures, cognitive behavioral therapy, and graded activity.

So if your fatigue is long-standing, step one isn't "tonify" — it's a thorough checkup, because behind a lot of fatigue hides one specific, treatable cause.

What TCM Sees

In TCM, "qi" is the body's energy currency, and fatigue means either spending more than you earn or circulation that's stuck. Three common patterns in practice:

  • Qi deficiency: energy production is low — weak voice, reluctance to move, sweating with slight exertion, constant tiredness. Approach: tonify qi, strengthen the spleen, raise yang.

  • Yang deficiency: the body's "furnace" runs cold — marked cold intolerance, icy hands and feet, can't get out of bed in the morning, loose stools after anything cold. Approach: warm yang and tonify qi.

  • Dual deficiency of qi and blood / liver stagnation with deficiency: long-term overdraft (pale sallow complexion, palpitations, insomnia), or stress-type fatigue (exhausted yet can't sleep, chest tightness, sighing, restless even while resting). Approach: tonify qi and nourish blood, or soothe the liver while supporting the upright qi.

TCM has a distinctive way of asking about fatigue: not just "are you tired," but "tired how" — tired in the morning or the afternoon, chilly or irritable when tired, wanting to lie down or unable to sleep even lying down. These details point to completely different treatment directions.

TCM also holds the spleen and stomach in special regard — "the spleen and stomach are the source of qi and blood production"; everything you eat must be transformed into qi and blood by them. In people with long-term dieting or aggressive weight loss, fatigue often starts with "not eating enough" — recovery begins with regular meals, not with more tonics.

Typical Presentations

  • Having to pause mid-sentence to catch your breath, palpitations and sweating on stairs, surviving the afternoon only on coffee — qi deficiency

  • Icy hands and feet year-round, worst in winter, feeling glued to the bed in the morning, tendency to loose stools — yang deficiency

  • Wiped out all day, yet the mind won't stop at night; chest tightness, sighing, more anxious the more you rest — liver stagnation with deficiency (stress-type fatigue)

How Acupuncture Helps

Acupuncture uses pattern-based point selection: qi-tonifying, spleen-strengthening points for qi deficiency; yang-warming points for yang deficiency; liver-soothing points added for stagnation patterns — aiming to regulate the autonomic nervous system and improve subjective fatigue and sleep quality. Some clinical studies suggest acupuncture may ease chronic fatigue–related symptoms, but most have methodological limitations and the evidence quality is limited. In clinical observation, people whose fatigue comes with insomnia and anxiety tend to respond relatively better to acupuncture, since one treatment can address several symptoms at once. To repeat: long-standing fatigue calls first for screening of thyroid, anemia, sleep apnea, and other organic causes; acupuncture is positioned as an adjunct after exclusion, or alongside conventional care.

5 Things You Can Do Daily

Keep a fixed schedule — sleep enough but don't linger in bed; 20–30 minutes of gentle daily movement (walking, Baduanjin qigong) — if you're still wiped out the next day, that's too much; eat enough protein at breakfast and cut sugary snacks (blood-sugar roller coasters worsen fatigue); no coffee after 3 p.m.; keep naps under 20 minutes — longer than 30 leaves you groggier, and a 5-minute walk beats coffee when drowsy in the afternoon; don't binge-sleep on weekends to "catch up" — it never repays a week's debt; before bed, write down three things you finished today — fatigued people tend to fixate on what's undone, which deepens mental drain.

Time to See a Doctor If…

Fatigue lasts 3+ months and rest doesn't fix it, especially with notable weight change, palpitations or chest pain, snoring with nighttime gasping, low mood, or noticeable memory decline. Get a full checkup to rule out organic causes first — bring the lab report, since doctors need the full trend of your markers; if nothing shows up and you're still exhausted, we'll look for the cause in your constitution and pattern together.
